Social Services
The Social Services Department provides patients and their families with counseling and referral services to help cope with the problems that often accompany an illness or hinder recovery and rehabilitation. As members of your health care team, we are devoted to helping people function the best they can in their environment.
Our Staff
The social services staff are experienced and dedicated professionals with masters degrees in social work and counseling. As members of your health care team, our staff can provide information and support as well as serve as a resource for services available in the community. Our staff can help ease the worries and concerns that can prevent patients from obtaining the maximum benefit from their medical care and can offer assistance and guidance to meet a variety of emotional and social needs such as:
- Adjustment to illness, disability or loss
- Advance Directives, information and guidance
- Behavioral health needs
- Child/Elderly protective concerns
- Domestic violence
- Discharge planning for patients needing rehabilitation or long-term care
- Family/Relationship issues
- Financial/Legal problems
- Referrals for basic needs such as housing, transportation and medications
- Substance abuse/chemical dependency
How to Receive Our Assistance
- Ask your physician, nurse or other hospital staff member.
- Call us between 8:00 a.m. and 4:30 p.m. Monday through Friday at 860-585-3269. There are no separate charges for our services.
